Can you Contribute to Labs & More for the Care of your Dog?




I understand that Labs and More is a Non Profit 501(c)3 Organization
Your Dog will need to be current on Vaccinations and altered or we require a donation to cover this expense. We ask that your dog be bathed and provided a current flea treatment. All Records will need to be sent to us prior to taking possession of Your dog. Please send any favorite toys, dishes, bed or other items that would benefit the care and comfort of your Dog.
By submitting this form, I hereby make available for adoption through Labs and More the Dog named above. I certify I am the owner of this Dog or have attempted to find the owner through appropriate channels. I Certify that my Dog is not possessed of any dangerous or vicious propensities and I have not willfully concealed information about my Dog. The Information provided is true and complete. I hereby forever release, discharge and agree to hold its board of Directors, foster home providers, Volunteers and agents from all claims, demands, actions, or liability of any kind as a result or in connection with the adoption of your Dog , and/or any harm or injury caused by your Dog as a result of offering your Dog for adoption, or whatever kind or nature. I further understand and acknowledge that: (1) Despite my offering my Dog for adoption, no such adoption may prove possible for my Dog because of either terminal illness causing pain or over-aggressiveness toward people that cannot be cured; (2) Labs and More may have no choice but to euthanize your Dog should either of those conditions obtain; and (3) my above release, discharge and agreement to hold harmless and indemnify CLRM, its Foster Home Providers Volunteers, and agents will also extend to, and cover, those events that may occur after I have delivered my Dog to CLRM for adoption through and until such time as my Dog is euthanized if adoption proves not to be possible for either of the above reasons.

Once you submit your application please send us at least 3 pictures of your dog to doginfo@labsandmore.com. Please allow 5-7 days for someone to contact you. You will then be asked to bring your Dog to a location for an evaluation. Once this is completed we will determine if we can help you find a new home for your Dog.
